TRAVEL INCENTIVE PROGRAM

TERMS AND CONDITIONS

Qualification Criteria

The 2017 Travel Incentive Program (“Incentive”) is open to all sales team members who are employed for the entire Qualifying Period.

The Qualifying Period: January 1, 2017 – December 31, 2017

Minimum Qualification Requirements:

Level 1 = 38 paid life and $16,000 paid premium (Average 3 life sold a month)

Level 2 = 48 paid life and $20,000 paid premium (Average 4 life sold a month)

Level 3 = 55 paid life and $24,000 paid premium (Average 4.5 life sold a month)

All life policies must be written and paid for prior to December 31, 2017 and must issue through underwriting before January 31, 2018.

Eligibility

The Incentive is open to all sales team members (“Members”) of Jared Soukup Insurance Agency who are 18 years of age or older.

Notification/Reward Acceptance

A confirmation/invitation will be mailed to the qualifier following the end of the Incentive Qualification Period.

Qualifiers will have 10 business days to provide written acceptance of their intent to redeem their Incentive reward with Jared Soukup Insurance Agency and JetSetPilot Travel, and sign a Waiver and Release for participation. JetSetPilot Travel will then confirm qualifiers participation and coordinate travel arrangements. Qualifying members will be responsible for securing their own travel visa/passport, if applicable

JetSetPilot Travel will determine departure airport locations.

Airline tickets are non-refundable/non-transferable and are not valid for upgrades and/or frequent flyer miles.

Hotel and Transportation

Qualifiers will be responsible for any transportation and other fees not explicitly outlined in individual incentive packages.

If the member is eligible Jared Soukup Insurance Agency will pay 100% of the cost of the trip for them and one qualified guest. Additional travelers travel at the qualifier’s expense on a standby basis, subject to available space.

Qualifier will be responsible for complying with commercial airline policies and any fees associated with luggage. Jared Soukup Insurance Agency does not reimburse for any parking, rental car or personal lodging costs.

The qualifier will be required to use their personal credit card for security deposit to cover hotel incidentals.

Travel and Trip Booking

Suggested booking window is 6-8 weeks prior to departure. Booking cannot be made less than 4 weeks prior to departure.

Travel cannot be planned for the following blackout dates: Easter, 4th of July, Memorial Day, Labor Day, Thanksgiving, Christmas, New Years and State Farm Ambassador Travel dates.  Additional travel dates may be restricted based on seasonal peaks and special events causing price hikes in hotels, airfare and cruise lines.

Qualifier must travel within 12 months of receiving notification mail.

All special requests or disabilities must be disclosed to JetSetPilot travel prior to the signing the final trip itinerary.

Availability and options cannot be guaranteed until final itinerary has been received and signed.

Once final travel itinerary has been signed by the qualifier all trip details are final and no modifications can be made to the trip. If changes are required, qualifier will be billed directly/accordingly.

Cancellation and Refunds; Force Majeure

The qualifier is associated with their Incentive reward and cancellations will not be refunded.

Travel Insurance will be available for the qualifier to purchase through JetSetPilot Travel. Jared Soukup Insurance Agency and JetSetPilot Travel will not be held responsible if cancellation of the event occurs due to any of the following: strikes, picket lines, boycott efforts, fires, floods, accidents, war (whether or not declared), revolution, riots, insurrections, acts of God, acts of government, acts of the public enemy, scarcity or rationing of gasoline or other fuel or vital products, inability to obtain materials or labor, or other causes that are reasonably beyond the control of Jared Soukup Insurance Agency or JetSetPilot Travel. No refund of an incentive cash equivalent will be provided.

Limitations of Liability

Jared Soukup Insurance Agency and its affiliates, subsidiaries, parent companies, advertising and Incentive agencies, printers, and legal entities involved in this Incentive are not responsible for any inaccuracies in information which may be used in the Incentive, for any technical or human error which may occur in the processing of qualification entries, including data entered by qualifiers, any entry miscommunications such as technical failures related to computer, telephone, cable, and unavailable network or server connections, related technical failures, or other failures related to hardware, software, or virus, or incomplete, late, or misdirected entries. Any compromise to the fair and proper conduct of this Incentive related to a computer virus or similar type of technical impairment that may affect the proper fairness, security, and administration of the incentive may result in the termination, amendment, or modification of the Incentive portion thereof at the sole discretion of Jared Soukup Insurance Agency. Jared Soukup Insurance Agency reserves the right, in its sole discretion, to void any qualification entries of qualifiers whom Jared Soukup Insurance Agency believes has attempted to tamper with or impair the administration, security, fairness, or proper play of this Incentive who acts in a non-sportsman like manner or with the intent to threaten or harass any other person.

By participating in the Incentive, Members waive all rights to claim punitive, incidental and consequential damages, legal fees, or any damages other than actual out-of-pocket costs or losses that may arise in connection with participation in the Incentive acceptance, possession, and use of any incentive reward.

Other Qualification Criteria

If aspects of the Incentive change outside the control of Jared Soukup Insurance Agency, neither Jared Soukup Insurance Agency, nor any other promotional partners furnishing incentive rewards will be liable or responsible for any such changes and Jared Soukup Insurance Agency reserves the right to substitute an incentive reward of equivalent or greater value, at Jared Soukup Insurance Agency’s sole and absolute discretion, if, for any reason, all or any portion of an Incentive reward, as described herein, are or become unavailable. No transfer of an Incentive reward to a third party is permitted. No cash substitutions will be offered for non-cash incentive reward. By accepting a reward, qualifiers agree that Jared Soukup Insurance Agency shall not be liable for any injuries, damages, or losses of any kind resulting from or in connection with the awarding, acceptance, possession, or use of the incentive reward or any aspects or parts thereof.

Jared Soukup Insurance Agency will be the final arbiter of all rule interpretations and qualification determinations and such decisions will be final. Where applicable, any qualification subsidy will be reported as income on the qualifiers annual tax return. Qualifiers will be responsible for paying all taxes on any incentive reward and/or subsidy or subsidies granted by Jared Soukup Insurance Agency. Jared Soukup Insurance Agency reserves the right to suspend or terminate this Incentive, without notice, if it deems necessary, in its sole discretion.

By participating in this Incentive, you (i) acknowledge compliance with these Incentive Criteria including all eligibility requirements, (ii) warrant that any information you provide in connection with this Incentive is true and accurate, and (iii) agree to be bound by the decisions of Jared Soukup Insurance Agency which shall be final and binding in all matters relating to this Incentive. Members who have not complied with these Incentive Criteria are subject to disqualification.
